Abstract: In spite of global difficulties caused by ecologic disturbances and financial problems and resulting in social crisis worldwide, the Editorial Board of Acta Dermatovenerologica Croatica (ADC) have managed - owing to their high enthusiasm - to publish all four Volume 16 issues. In 2008, we have received due moral satisfaction for our efforts, i.e. inclusion in the Web of Science database, as an additional impetus for the future. We will try to reach the peak by the journal inclusion in Current Contents. Shall we manage to do it? Thirty-four articles have appeared in the ADC volume 16, including 4 papers submitted by young researchers published in the Medical Research section and 5 scientific papers. Thirteen international and domestic scientific and professional meetings have been presented in the News section. The papers submitted for publication were evaluated by 73 reviewers, which also reflects our efforts invested in the appearance of ADC volume 16 with 236 pages. Some new ideas have been considered for volume 17, however, the 21st century appears to be characterized by less enthusiasm in our junior fellows; they seem to be ready to take certain activities just to a measure required for their academic career, no more and no less than that. Therefore, the Editorial Board members make great efforts to stimulate quality publications. Regular four issues with topics related to research activities may be questionable, in spite of all efforts made by the Editorial Board. We will welcome more Letters to the Editor, more research reports from pharmaceutical industries with a Guest Editor, and ADC issues dedicated to specific topics of interest. Such an issue on the topic of psoriasis has just been prepared. It is quite difficult to find reviewers for particular scientific papers because this valuable engagement is voluntary and not properly validated. A decision from the Croatian Medical Chamber on the scoring system for reviewers has been expected for quite a long time now. Such a document would certainly facilitate the process, as reviewers would be more ready to do it knowing that it will be properly recognized, not just as moral satisfaction through the Editorial Board's appreciation of their high scientific competence. We are grateful to all our reviewers for the endeavors they have invested in ADC volume 16, and we do hope for their precious collaboration in ADC volume 17. Yet, the Editorial Board feel satisfied with the achievements of ADC volume 16. The level of the journal has been upgraded, as indicated by the ascending curve from the year 2006 impact and we hope for this tendency to continue with ADC volume 17. Although the prognosis for 2009 is not promising, I believe that we will manage to fulfill the set goals by our joint efforts and hard work. I also call you to join us with your highly welcome and valuable contributions.

Abstract: For over 30 years, investigators have used the simple but non-validated classification criteria suggested by Moll and Wright. Several authors have suggested modifications to these but most remain unvalidated or require human leukocyte antigen analysis. Now, a worldwide initiative has developed new criteria which include both clinical and radiological features. These will require further study before they are fully adopted but their improved performance should result in less variation between study cohorts. The recurring question of disease heterogeneity continues to occupy researchers in this field. Despite recent pleas to abandon the original five sub-groups, a case can be made for retaining at least the two sub-groups of peripheral and axial disease and, possibly, splitting the peripheral disease into oligo- and poly-arthritis.

Abstract: Psoriasis plaques that have very thick adherent scale (so-called coral reef or rupioid psoriasis) tend to be resistant to topical treatment. We present a case of coral reef psoriasis that responded to topical clobetasol 0.05% spray without concomitant use of moisturizers or keratolytic agents. We propose that coral reef psoriasis is resistant to topical corticosteroids because of poor adherence to treatment, not because of poor penetration of applied agents.
